Pain care

Weekend appointments for shift workers and caregivers

Nurses, drivers, warehouse staff and family caregivers postpone pain care most. Saturday and Sunday scheduling is built around them.

Two groups of patients delay care longer than anyone else in our practice. The first works shifts that overlap every clinic's business hours. The second spends weekdays caring for someone else and has no window left for themselves. Both usually arrive with pain that has been running for months.

Why the delay is costly

Pain that has been present for a long time is generally harder to unwind than pain caught early, and the surrounding problems accumulate: sleep loss, deconditioning, altered movement patterns that create a second problem in a different joint, and the mood effects of chronic pain. A plan that would have taken six weeks earlier can take considerably longer later.

Delay also narrows options. Some conservative approaches work best before compensation patterns are entrenched.

How the weekend block is structured

Saturday and Sunday run 8:00 AM to 4:00 PM at the Silver Spring office, with full evaluations rather than abbreviated visits. Early Saturday slots are the most popular with night-shift workers coming off a Friday overnight; midday Sunday tends to work best for families bringing an older parent.

Weekday evening telehealth from 5:30 to 8:00 PM covers follow-ups, so a completed course of care may only require one or two weekend trips.

Bringing someone with you

Caregivers are welcome in the room with the patient's consent, and we encourage it for older adults. A second set of ears catches instructions, and a caregiver often supplies the functional history — what the patient can no longer do at home — that the patient underreports.

Preparing for a single weekend visit

Complete the intake beforehand. Bring the medication list, prior imaging reports and any therapy notes. Write down the top two limitations you want addressed. If travel is difficult, ask about house calls or whether the follow-up can be telehealth. Coming prepared is how one weekend visit becomes a complete plan instead of a first step.

Cost

The self-pay evaluation is $175 and most major insurance is accepted, with the same pricing on weekends as weekdays.

Frequently asked

Are weekend slots limited?

Weekend capacity is finite and books earliest in the week, so scheduling in advance is recommended.

Can a family member attend the visit?

Yes, with the patient's consent. For older adults we encourage it.

What if I cannot travel to Silver Spring?

Telehealth is available across Maryland, and house calls can be discussed for patients who cannot travel.
